Who could imagine that a treadmill accident, of all things, led to the shocking death of a top Silicon Valley executive?

Dave Goldberg "fell off the treadmill and cracked his head open" according to authorities in Mexico where he was vacationing with his famous wife, Sheryl Sandberg. She is second-in-command at Facebook and author of the best seller Lean In.

The accident happened at a luxurious resort in Punta Mita.

Dave Goldberg reportedly left his private beach-front villa around 4 p.m. Friday to work out at a gym.

About three hours later, when he hadn't returned, his brother went looking for him and found him on the floor of the gym in a pool of blood.

He was rushed to a hospital, but died of head trauma and blood loss.

There were over 24,000 treadmill injuries reported by hospital emergency rooms in 2014. YouTube is chock-full of apparent treadmill accidents.

Fitness expert Steven Zim, owner of the gym A Tighter U in Los Angeles showed us the right way to use one.

Here are the mistakes to avoid on a treadmill:

  1. Never use a cell phone.
  2. Stop the treadmill if you need something as simple as taking a sip of water.
  3. Wear proper shoes.

Zim told INSIDE EDITION, "When people are texting or e-mailing on the cell phone, they're distracted and they're not focusing. They can trip; If you trip you can hit your head. Even if you are getting off for one second, come to a complete stop. A good shoe is rolled in the front, rolled in the back, you can go heel-to-toe."
